Introduction Intravenous fluids are a routine prescription for the majority of emergency surgical admissions. The recent British Consensus Guidelines on Intravenous Fluid Therapy for Adult Surgical Patients (GIFTASUP) issued by (amongst other specialist organisations) the Association of Surgeons of Great Britain and Ireland, The Intensive Care Society and British Renal Association (Powell-Tuck et al 2008) represent comprehensive, evidence-based and accessible guidance regarding the use of intravenous fluids. In particular, they incorporate the recent expansion in evidence highlighting the often preferential characteristics of balanced crystalloids such as Hartmann's solution. These solutions have more physiologically desirable characteristics than unbalanced solutions such as 0.9% saline, and consequently reduce adverse outcomes including hyperchloraemic metabolic acidosis, splanchnic hypoperfusion and sodium and volume retention, which may be a consequence of 0.9% saline administration (Wilkes et al 2001).
